Insurance Information

The Center for Women’s Health participates with most insurance companies.  We contract with all Blue Cross Blue Shield plans, Cofinity, Cigna, United Healthcare, Pacificare, Banner, Great West, Rocky Mountain Health Plans, MultiPlan, and Private Health Care Systems.  If your company or network is not listed, please check with them directly to verify that we are in their network.  Some companies require us to bill all claims under Dr. Abbot so he may be the only provider listed. 

We are happy to submit your insurance claims for you.  Each time you come in for your appointment, please bring your current insurance card with you so we can get a copy of it for billing purposes.  We will also need the policy holder’s social security number and date of birth as may be required by your insurance company.  Please plan on paying your copay and/or coinsurance when you check in. We ask you to keep us informed of any changes that transpire between appointments including a new address, phone numbers, and insurance change.

Most of our lab specimens are obtained here and sent to an outside facility for testing.  Many insurance companies have contracts with specific laboratories and will not pay for services performed at a facility with which they are not contracted.  Please inform the medical assistant if this is the case so they are able to facilitate these requirements.

Some services may not be covered by your insurance.  If your claim is denied or not covered according to your policy benefits, please contact them directly.  We cannot change diagnosis codes in order to conform to your benefits.  Payment for these services then becomes your responsibility.

We are happy to assist you with any insurance issues or questions you may have.  Our primary goal is to provide you with the highest quality health care, and with your cooperation and assistance, you should be able to receive the fullest benefits offered to you.
