DigiChart Health History Website

We have converted to electronic medical records, and you have the privilege of being part of that process!  In order to enhance the level of service we provide you, we require that you complete your health history online at least 24 hours prior to your next appointment.  The link is https://patients.digichart.com/.  The website is encrypted and very secure to ensure the protection of our patients’ private information.  It requires a username and password that will be sent to you after you schedule your appointment. 

Once you have received your username and password just click on the link above to get started.  You may be asked to change your password when you first login so please write it down for future access.  If you are interrupted while entering your information, you can return to the website at your convenience and pick up where you left off.  It may take you 30 minutes to enter your history, but it will be very helpful for your provider to have all of your information in your chart when you arrive for your appointment.
